BizTalk FILE Adapter Data Duplication Issues over UNC / Shared Path

Hi All

Our environment has two Production BizTalk 2009 servers in Active/Active mode clustered over SQL Instance DB 2008 server. We're using FILE Adapters to send and receive data from a Shared UNC Path on a separate third server (FILE Server) for High Availability. We mostly use legacy adapters such as FILE, SMTP, etc in our applications

Everything was working fine for 6 months until last week when suddenly random data started getting duplicated. Due to multiple servers, multiple BizTalk Applications, etc being involved it has become difficult to trace the Root Cause. The possibilities that we could think might be occurring are:

  1. File locking issues for the FILE Adapter Receive Ports over UNC/Share Path due to Active/Active BizTalk servers (but unable to comprehend why this suddenly appeared after 6 months and that too with only one particular BizTalk Application; possible clues: this application is a data intensive application with a lot many more transactions and was deployed one month before this issue started surfacing [coincidence?] )
  2. File locking issues for the FILE Adapter Receive Ports over UNC/Share Path due to Network Latency or Disconnect/Issues (could this be due to increased load on the servers?)
  3. Memory issues due to the increased load on the FILE Server (read in someone's blog)
  4. Any other possibility?

I've read the known issues of FILE Adapter provided by MS (http://msdn.microsoft.com/en-us/library/aa560642.aspx), what could be the possible reason of the file locking protocols being breached here? Is there any other possibility that we might be missing?

Even more so, could someone suggest how to diagnose the root cause and take preventive measures? (Also studied/been studying Troubleshooting: http://msdn.microsoft.com/en-us/library/aa561768)

Please help
